Join Partners in Healthcare

To become a member, your agency representative will need to submit our Partnership Application Form.

Upon submission, the Partnership Committee will contact you to complete the process. Eligible applicants will be required to:

  1. Submit signed copy of the Partners Individual Networking Agreement to the Partnership Committee.
  2. Coordinate with the Hospital In-Service Coordinator for scheduling presentation dates with hospitals; by making your selections in your Partners Online Account.
